Why it is autoinflammatory and not autoimmune
Repeatedly negative ANA, rheumatoid factor and anti-dsDNA are not an absence of evidence. They are the evidence. Her adaptive immune system is not attacking her, and that rules out an entire category of disease.
What remains is the innate arm: abrupt, stereotyped, fever-driven flares with sterile cultures and no antibiotic response. A hair-trigger pyrin inflammasome floods the system with IL-1β, serum amyloid A follows, and amyloid deposits. One pathway, twenty clues.
AUTOIMMUNE
The adaptive arm, targeting self
Autoantibodies and self-reactive lymphocytes. Serology is positive, and onset is usually gradual.
AUTOINFLAMMATORY
The innate arm, firing without a target
No autoantibodies. Abrupt stereotyped fevers, sterile inflammation, and a response to IL-1 blockade rather than to immunosuppression.
