Hepatic Portal System

The hepatic portal system includes veins from the stomach, intestines, spleen, and pancreas. These veins carry nutrient-rich blood directly to the liver via the hepatic portal vein.  Remember that a portal system is a one-way connection between organs. It is similar to the hypophyseal portal system that we saw in the endocrine system.  Here, all the organs absorbing nutrients are connected to the liver via this hepatic portal system.  The liver processes and detoxifies the blood. It metabolizes nutrients and stores vitamins and minerals. It also breaks down harmful substances like toxins and medications.  Yes, medications.  Most medications you take are considered toxins by the body. The liver and the kidney try their best to clear them out. Still, you keep taking them!  Once the blood is clean, I discuss this in more detail in another mini-lecture. The liver dumps the blood right into the vena cava. It then goes to the right atrium.  Yes, the spleen is also attached to the hepatic portal system.


Ascites

Ascites refers to the abnormal accumulation of fluid in the abdominal cavity, often caused by cirrhosis or severe liver disease. Cirrhosis can develop due to chronic hepatitis, fatty liver disease, or alcohol abuse. It leads to scarring of the liver tissue. This scarring disrupts normal blood flow. This impaired circulation increases pressure in the hepatic portal vein. This condition is known as portal hypertension. It causes fluid to leak into the abdominal cavity. Ascites can result in swelling, discomfort, and difficulty breathing due to pressure on the diaphragm. Treatment typically involves managing the underlying liver disease, draining the fluid, and sometimes using medications to reduce fluid buildup.


Liver Diseases

Hepatitis is an inflammation of the liver caused by the hepatitis virus. There are different types of hepatitis viruses (A, B, C, D, and E). Hepatitis B and C are particularly concerning. They have the potential to lead to chronic liver disease. Chronic hepatitis can cause long-term damage to liver cells, leading to scarring (cirrhosis) and an increased risk of liver cancer. Symptoms of hepatitis often include fatigue, jaundice (yellowing of the skin and eyes), abdominal pain, and dark urine. Early diagnosis and treatment are essential to prevent further liver damage and complications.

Fatty liver disease, or hepatic steatosis, occurs when fat accumulates in liver cells. This condition can develop due to excessive alcohol consumption, known as alcoholic fatty liver disease. It can also result from factors such as obesity, diabetes, and high cholesterol, referred to as non-alcoholic fatty liver disease. Fatty liver disease can range from mild to severe. In its early stages, it may not cause noticeable symptoms. However, over time, it can progress to inflammation. It can also lead to liver damage and cirrhosis. Non-alcoholic fatty liver disease is particularly concerning. It is often linked to metabolic syndrome. If left untreated, it can lead to complications such as cardiovascular disease and liver failure.
