MODULE 21 · CONCLUSION & ASSESSMENT
Not bad luck. A diagnosis.
You now hold reproductive anatomyThe study of the structure of the human body., gametogenesis, the ovarian and uterine cycles and early pregnancy. Here two separate workups are read as one disease that had already inflamed the pelvis.
Why neither workup found the cause
Her bleeding was investigated as a gynecologic problem and her infertility as a separate misfortune. Both investigations were competent, and neither asked what else had been inflaming her peritoneumThe membrane lining the abdominal cavity and organs. for thirty years.
Recurrent peritonitis leaves adhesions, and adhesions distort the structures a pregnancy depends on. Chronic inflammation also disturbs the hormonal axisSecond cervical vertebra; has the odontoid process (dens) for pivoting head (“no” motion). driving the cycle. Two mechanisms, one disease, both invisible to a workup scoped to the pelvis.
UNEXPLAINED INFERTILITY
A diagnosis of exclusion
Reached when the pelvic workup is normal. It describes the limits of the search, not the state of the patient.
INFLAMMATORY SEQUELAE
A cause that was never in frame
Adhesions from recurrent serositis plus a disturbed hormonal axis — visible only if you look outside the pelvis.
