Stina almost didn’t mention the fog. When a new clinician finally ordered a brain MRI, the report came back with a phrase she had to look up: white matter hyperintensities — bright flecks scattered through the brain’s wiring, the radiologist noted, “more than expected for her age.” The suggested explanation was a shrug in medical language: probably small-vessel changes, or maybe migraines.

To understand why those spots matter, you first have to know what the brain is and how it is normally shielded. The brain is the body’s control centerThe part of a feedback loop that processes information and initiates a response. — billions of neuronsThe functional cells of the nervous system that transmit signals. organized into regions, wired together by myelinated tractsBundles of nerve fibers in the CNS that carry signals between brain regions. that make up the white matter, and protected by one of the tightest barriers in the body. On MRI, a hyperintensity is simply tissue that lights up too bright, usually where myelin or small vessels have been injured. A few such spots are common with advancing age; a young or middle-aged brain carrying more than its years warrants is a flag, not a footnote.
