The same echocardiogram that found the fluid around Stina’s heart also showed the heart itself working normally: four chambers filling and emptying, four valves snapping open and shut in sequence. The technician traced the structures aloud — right atrium, right ventricle, left atrium, left ventricle — and Stina, who had spent decades being told her symptoms were ‘nothing,’ found it oddly reassuring to watch the pump that had carried her through every flare keep its steady rhythm.

The heart is really two pumps fused side by side. The right side receives oxygen-poor blood from the body through the venae cavae, fills the right atrium, drops it through the tricuspid valve into the right ventricle, and ejects it through the pulmonary valve into the pulmonary trunk toward the lungs. The freshly oxygenated blood returns by the pulmonary veins into the left atrium, passes the mitral (bicuspid) valve into the muscular left ventricle, and is driven through the aortic valve into the aorta to supply the entire body. Atrioventricular valves stop backflow into the atria; semilunar valves stop backflow from the great arteries. This circuit is the stage on which everything else in the module happens — and the pericardial sac that inflamed in Stina is simply the wrapping around this whole two-sided pump.

For Stina, the four-chamber tour is quietly reassuring and quietly important. The same echo ordered for her effusion imaged a structurally normal pump — four chambers filling and emptying, four valves opening in sequence, a left ventricle three times thicker than the right doing its high-pressure job without complaint. The fluid that brought her in was not inside this engine but outside it, in the sac that wraps the whole thing. Building the pump is really building the backdrop against which one inflamed lining stands out.
