Ask most people where digestion happens and they point at the stomach. But the digestive system is not a set of separate organs so much as one continuous tube – roughly nine meters of it – running without interruption from the mouth to the anus. Food never truly enters the body until it is absorbed across that tube’s wall; until then it is traveling through a passage that is, in a sense, still the outside world. Understanding Stina’s attacks means understanding how that tube is built, because the layer that hurts is not the one that touches her food.

From the lumen outward, the GI tract wall has four layers. The mucosa lines the lumen and does the absorbing and secreting. The submucosa beneath it carries blood vessels, lymphatics, and nerves. The muscularis externa, usually two sheets of smooth muscle, produces the peristalsis that pushes food along. And the outermost layer, the serosa, is a thin, slick membrane – continuous with the peritoneum, the lining of the abdominal cavity itself. The gut also runs much of its own show: the enteric nervous system, a web of over a hundred million neurons embedded in those walls, coordinates motility and secretion so independently that it is nicknamed the ‘second brain.’ For Stina, the layer that matters is the outermost one. Her pain is not coming from her mucosa or her muscle; it is coming from the serosa and the peritoneum it joins – a membrane that, when inflamed, can make the entire abdomen rigid and tender. Hold that layer in mind; it is where this module ends.

For Stina, the four-layer wall points straight at the suspect. Three of the layers — the mucosa, submucosa, and muscularis externa — belong to individual organs, but the outermost, the serosa, is in the abdomen continuous with the peritoneum, the serous membrane wrapping every organ at once. A whole-abdomen attack with fever and no infection isn’t the lining of one organ misbehaving; it’s the shared serous wrapping inflaming across the whole belly.
